How do I refer?

Please click on the link below to complete the online referral form. When you click submit, this will automatically send it to our admin team. We will contact you once your referral form has been processed. We may have additional questions about your referral. If so, we will contact you prior to your referral form being accepted. You must provide as much information on the referral form as possible, failure to do this may result in your referral being rejected. Please note, once you have started the referral form, there is no option to save and continue at a later date. You must complete the form in one session.

At the ADHD Foundation Neurodiversity Charity, we offer a full pre-diagnostic ADHD screening assessment for 18- to 60-year-olds. This is not a formal diagnosis, it is a comprehensive screen used to support a diagnosis comprising of the QbTest or QbCheck, ADHD rating scales, a clinical interview and full history. This is a useful tool to determine whether ADHD may be present and create a body of evidence that can be used by a diagnosing clinician.

 The price for a full assessment is £450. A QbTest only costs £300.

Once you have received the results of your pre-diagnostic screening, and if you would like to continue your assessment and pursue a formal diagnosis, we can refer you over to a private psychiatrist. We work in association with several private psychiatrists who can use the evidence provided from the pre-diagnostic screening assessment to continue the diagnosis. They charge an additional fee £450 for a diagnostic consultation, and £140 for each follow-up/medication review appointment required. Alternatively, we can send the results to any healthcare provider of your choice. This can be discussed following the pre-diagnostic screening.
